If your building fits any of the descriptions on the left, it is required to have a Combined Fire Safety and Emergency Action Plan on file with the FDNY (Section of 401.4 of the NY Fire Code).
This plan is the most comprehensive plan required by the FDNY, it is considered Level 1. You are required to have a Fire and Life Safety Director and additional FLS staff to act as building stewards during emergencies and drills. Procedures must be listed for reporting emergencies and coordinating with emergency services. Plan of actions must be developed for various non-fire emergencies, accounting for building occupants, and attending to persons with special needs.
A written description of the building and its appurtenances must be developed, in which the building elements such as stairways, elevators, exits, fire alarm boxes, emergency power, etc. must be documented.
Riser diagrams are required for the standpipe and sprinkler systems.
Floor plans of each level of the building must be developed to the FDNY standards.
A fire safety maintenance program must be developed and persons responsible for maintaining the building systems must have their certificates of fitness.
A Building Information Card must be developed for the FDNY first responders.
A high-rise or large-area Group B office building occupied or designed to be occupied by more than five hundred persons, or more than one hundred persons above or below the street level, without a fire alarm system.
a Group B office occupancy occupied or designed to be occupied by a total of more than one hundred persons on one or more high-rise floors without a fire alarm system.
any other Group B office building, and any other Group B office Occupancy, occupied or designed to be occupied by more than five hundred persons, or more than one hundred persons above or below the street level, that was required to have a fire safety director under the New York City Fire Prevention Code or this code prior to the effective date of this section.
any other Group B Occupancy or occupancy as to which the Commissioner determines that preparation of such a plan is in the interest of public safety given the location, use or Occupancy of the building.

Assembly spaces of over 5,000 persons
Educational spaces in high rise buildings with voice communication capability or where 100 or more persons occupy one floor of the high rise building.
High rise mercantile buildings, a mercantile building with 300,000 sqft of floor area or more, a covered mall of over 300,000 sqft.
